About Community Connections

The Community Connections program offers short-term, tailored support for people at risk of social isolation. It helps individuals:

  • Identify and overcome challenges
  • Set and achieve personal goals
  • Strengthen wellbeing by building stronger community connections

The team provides information, guidance, and practical help, as well as referrals to local groups, training, volunteering opportunities, and support services. This assistance empowers people to explore their interests, boost confidence, and participate more fully in their community.

Community Connections is funded by the South Australian Government’s Department of Human Services.
