About the Role
This is a 11-month fixed term contract position, covering Parental Leave.
As the Community Support Officer, you'll play a vital part in supporting older members of our community to live independently and stay connected through Council’s Commonwealth Home Support Program.
You’ll work directly with clients accessing our Positive Ageing services, coordinating supports, maintaining accurate client records and ensuring services are delivered efficiently, respectfully and on time. Your work will help ensure clients’ needs are clearly understood and the right services are in place to support their wellbeing.
You’ll also contribute to the smooth operation of the program by supporting day-to-day activities, coordinating service provision and managing information with accuracy and care. Working in a fast-paced, community-focused environment, you’ll use sound judgement, follow established policies and respond confidently as priorities shift.
Key responsibilities
- Support the effective delivery of the Commonwealth Home Support Program (CHSP) in line with funding, compliance and organisational requirements
- Conduct client assessments and reviews, developing individual service plans that incorporate goals, wellness and reablement approaches
- Coordinate timely, person‑centred service delivery to meet assessed client needs
- Support clients and community members to navigate My Aged Care intake, referrals and service pathways
- Deliver high‑quality, person‑centred customer service to clients, carers and community members
- Assist in managing complex or sensitive interactions with clients, service partners, contractors and volunteers
- Maintain accurate, confidential and compliant client records using client management systems and corporate technology.
About You
To be successful in this role you will have:
- Relevant qualification (to Diploma level) and/or experience in coordinating community‑based services
- You are empathetic, respectful and client‑focused, with a genuine commitment to supporting older people in the community
- You bring a strengths‑based, wellness and reablement approach to your work
- You are organised and detail‑focused, able to manage competing priorities in a dynamic environment
- You demonstrate sound judgement, professionalism and discretion, particularly when handling sensitive information
